Premium Disability Care Services

Group / Centre Activities

We offer the necessary support so you can engage meaningfully in social or recreational activities such as sports or art classes. Centre-Based Activities and Groups can encourage healthy socialisation while at the same time offering capacity building activities. Participants are welcome to take part independently or in facilitated workshops.

Community Participation

We empower participants to become actively involved in recreational activities within the community as well as broader community participation. For example, going on camps, community events or vacations. We understand how important it is to engage with your community in order to build relationship skills, confidence, and overall self-development.

Household Tasks

Our helpful team can assist you with meal preparation, general cleaning, yard maintenance, doing the washing and drying or getting the grocery shopping done. Depending on your preference, we can complete these tasks for you or simply guide you towards doing them independently with more ease.

Development Life Skills

This refers to assistance with, or supervision of personal tasks during day-to-day life that will enhance your ability to live as independently as possible. Our friendly carers can help in your own home, shared living or in the community. This includes personal care like showering and toileting, doing the shopping, managing bills, taking public transport, and problem-solving skills.

Innovative Community Participation

Innovative Community Participation is a capacity building support. By participating in community projects such as volunteering at a charity or getting involved with a local organisation, you can gain confidence, acquire new skills, and increase awareness of social norms. It may also increase your chances of employability. Our friendly support staff will help you connect with programs in your community and facilitate your participation.

Daily Tasks Shared Living

Daily living funding falls under the category of Core supports within the NDIS, and includes customised support if you are living in a shared accommodation or with a host family. We assist with things like personal hygiene, getting dressed, mobility in and out of bed, eating and drinking, or going to your appointments.

Assist Travel / Transport

We help you to get around your community with ease and comfort. One of our support staff members will drive you safely to your appointments, to do your shopping, or take you to a community event. This way, you have a reliable, trusted mode of transport whenever you need it. We can also equip you with the skills to use public transport independently.

Community Nursing

Our nurses at Premium Disability Care and Support are highly skilled and fully qualified to provide dignified nursing support with respect and sensitivity for each of our participants. This includes wound care, general health check-ups, overnight nursing care and respite, medication administration, after-hospital care and more.

Assist Personal Activities

Receive one-on-one assistance with personal activities such as basic household tasks, personal care, organising your home, overnight assistance, physical mobility, or communicating with friends and family. We understand that your need for personal assistance may be greater if you are living alone, so we provide familiar support for you to feel secure and capable of living life on your terms, at your own pace.

Assist Life Stage Transition

As you go through pivotal life stages and transitions such as starting a new job or moving into a new apartment, we will be by your side, ensuring it happens as smoothly as possible. If you find yourself in a situation that requires financial planning, conflict resolution, self-care or decision-making, we can assist you through these transitions by offering support and mentoring. We can also help connect you with your broader community and support providers to receive help with complex issues.

Support Coordination Level 1 & 2

At Premium Disability Care, we believe that everyone deserves individualised care and support services. That's why, in addition to our core supports and capacity building services, we also offer NDIS Support Coordination Level 1 and Level 2. Level 1 Support Coordination gives you an understanding of the NDIS and helps you access the services you need. Level 2 Support Coordination goes a step further and provides more intensive support to help you navigate the complexities of the NDIS and develop a plan that works best for you. We'll work with you every step of the way to identify your goals and needs, find the right support and services, and track your progress towards a more fulfilling life. Whether you're self-managed, plan-managed, or agency-managed, we're here to help you get the most value from your NDIS funding.

Assist Personal Activities High

We provide assistance with high intensity daily activities such as diabetes management and feeding, complex bowel management, catheter care, tracheotomy care and assistance with injections. To us, you are an individual who deserves the best care and support for your unique abilities. Our caring staff will help you manage your daily routine so you can continue to live as independently as possible.

Accommodation & Tenancy

We offer dependable assistance with finding safe, appropriate accommodation. Whether you need to retain your current accommodation or secure a new place to stay, we can support, guide and help you undertake any activities necessary to do this, such as applying for a new tenancy contract or fulfilling tenancy obligations.
